Purpose:

This record series consists of current health enrollment files maintained by the Office of Insurance/Health, Life, and Retirement.  The files contain all CMS health enrollments and corrections (e.g. change of name) for university employees including:  beneficiary designations, health certificates, and dependent affidavits.  Originals are maintained with the Department of Central Management Services with the exception of original beneficiary designations which are retained in office.
 

Procedure:

Retain in office for six (6) years following termination of coverage, then dispose of provided all audits have been completed under the supervision of the Auditor General, if necessary, and no litigation is pending or anticipated.
