Purpose:

This record series consists of an alphabetical listing of employees and payment of their insurance premium per pay period.  A copy of this report is sent to the Insurance Office for retention.
 

Procedure:

Retain in office for two (2) years, current and prior fiscal year, then dispose of provided all audits have been completed under the supervision of the Auditor General, if necessary, and no litigation is pending or anticipated.
